What is Breast Augmentation, Breast Implants, Breast Lift and Breast Fillers?

Before you visit your plastic surgeon, you may wish to understand a bit more about the meaning of breast implants, breast lift and breast augmentation in order to have a better conversation with your cosmetic surgeon.

Breast augmentation alone cannot correct sagging or drooping breasts. Simply increasing the volume, or size, of the breast will only cause the breast to sag more with time. When performed as a standalone procedure, breast augmentation can increase or balance the size of the breast, restore the breast volume, and restore the shape of the breast after partial or total loss. In contrast, during a breast lift procedure (mastopexy), loose, excess skin is removed and the remaining skin is tightened. As a result, the overall shape of the breasts is improved, and they are lifted to a new, more upright position. In combination, breast implants and a breast lift result in total rejuvenation, not only increasing the size of the breast, but restoring a firmer, perkier appearance as well.

Hyaluronic acid has come to represent the most widely used injectable cosmetic product in the world. Brought into being by the Swedish company Q-Med, in 2007 Macrolane™ was authorized for use in France, and the year after, it received official European approval as a means of breast augmentation. Since then, however, numerous controversies pertaining to its side effects have led to its withdrawal from the worldwide breast augmentation market.

Macrolane is a very new product which has been developed in Europe specifically for breast augmentation based on the concept of dermal fillers that we use today. It is an HA filler approved in Europe and the UK. The product is used to correct contour defects in the breast, and for augmentation, and like all hyaluronic acid gels will last up to a year or two before it has to be repeated. I have no information yet as to cost, or possible cup size. Also the complication and satisfaction rate is not known. It seems unlikely that the product will provide the volume possible with a saline, or a silicone gel breast implant. An implant currently would be the best alternative for most seeking enhancement.

Yes, I do that but not with hyaluronic acid type filler for it would cost too much but I use saline that I slowly inject into the breast as an office visit procedure. I fill the breasts up to whatever size the patient wishes. This slowly does disappear over a day or so but gives the patient an idea of what she will look like as well as maybe even helps her fill out her wedding dress for that special day! My patients like it and I get a better idea of what size implant they would prefer. I have been doing the same for cheek and chin implants for many years.

Hyaluronic acid is currently being investigated as a possible alternative to breast implants, but is not approved for general use as of yet. Saline injected into the breast would absorb very quickly and would not be effective.

How much does a breast augmentation cost in Malaysia?

As you can see below, the cost of breast augmentation may vary from RM 20,000 to RM 25,000 depending on various factors.

Fat transfers to the breasts may cost around MYR 19,000 to MYR 23,000 per session while breast implants may cost approximately MYR20,000 to MYR 25,000 in Malaysia.

Do take note that fat transfer to the breasts basically involves 2 procedures i.e. liposuction and breast augmentation. Double check with your plastic surgeon on the total cost and the breakdown.

The total cost would normally include the plastic surgeon’s fee, the operating room cost, the anaesthesia fee and any special charges (such as breast implants or extra procedure such as breast lift) and also the necessary follow up visit/s.

Note:
While the most expensive surgeon may not necessarily be "the best", there also may be reasons for a surgeon to charge a relatively lower fees. If "shopping" by phone for the "best deal" in plastic surgery, beware of "low-balling". The low-balling technique is a persuasion tactic in which an item is initially offered at a lower price than one expects in order to get the buyer to commit; then the price is suddenly increased.

Make sure you know what you are quoted; a surgeon’s fee vs. the TOTAL fee for the procedure. The total fee is higher since it would include the surgeon’s fee, the operating room cost, the anesthesia fee and any special charges (such as implants or laser fees) and also the necessary follow up visit/s. In general, the costs for each procedure are a general range for the surgeon's fee, the hospital / surgical center facility fee, and the fee for the anesthetist or anesthesiologist.

Thankfully, the Malaysian Government has come out with guidelines on aesthetic medical practice to address some of the confusion. It's clearly stated in the guidelines that breast implant procedures are major procedures and have to be performed by a qualified plastic surgeon (and not by a beautician) in an operation theater at a hospital or a licensed ambulatory care center (ACC).

Moving Forward - What Next?

You may wish to run through the following as your summary checklist:
  • Filter and shortlist your clinics.
  • Once you have shortlisted the clinics, book an appointment to have a consultation with your plastic surgeon. Do compare them based on your first impressions and other considerations. Use a table with factors to consider and a scoring for each important factor, if you need to improve your clarity.
  • Are they accommodating in terms of appointment time and date?
  • Was a solid plan of action determined with detailed information especially post-procedure precaution and care?
  • In the event of a possible complication or infection, do you need to pay extra?
